As a courtesy to our referring offices, we are happy to assist with pre-certs for most insurances by your request.
We ask for 2 full business days in advance to obtain the pre-cert before the patient can be seen. We cannot pre-cert same day patients. If a patient needs to be on the schedule for same or next day, the referring office will need to arrange for the pre-cert to be completed before the patient's appointment.
Our goals are to help our referring offices, schedule our patients promptly, and keep the pre-certs from having to go to review. In order to be successful, we MUST HAVE the following information faxed to us:
- Imaging order
- Copy of patient's identification
- Copy of patient's insurance card
- Office notes
- Information of any previous imaging testing (insurance requires this!)
- Patient demographic sheet
- Contact person in your office and their phone number
Our fax number is 859-276-1540. Some insurances will only allow us to initiate the pre-cert process and require the referring office to complete it.
